Zang Fu patterns (Tips)


HEART PATTERNS:

HEART EMPTY PATTERNS:

Heart-Qi Deficiency: palpitations, tiredness, empty pulse.

Heart-Yang Deficiency: palpitations, feeling of cold, cold limbs, deep pulse.

Heart-Yang Collapse: cyanosis of lips, hidden‐minute pulse, cold limbs.

Heart-Blood Deficiency: palpitations, insomnia, poor memory, pale tongue.

Heart-Yin Deficiency: palpitations, mental restlessness, feeling of heat, malar flush, red-peeled tongue, deep midline crack.

HEART FULL PATTERNS:

Heart-Fire Blazing: tongue ulcers, thirst, palpitations, red tongue.

Phlegm-Fire Harassing the Heart: tendency to hit or scold people, uncontrolled laughter or crying, agitation, shouting, muttering to oneself, mental depression and dullness; in severe cases aphasia and coma.

Phlegm Misting the Mind: mental confusion, rattling sound in throat, sticky-slippery tongue coating.

Heart-Qi stagnation: Palpitations, feeling of oppression or distension of the chest, depression, slight lump in the throat, slight shortness of breath, sighing, poor appetite, dislike lying down, weak cold limbs, pale, purplish lips. Tongue: Pale-purple. Pulse: Empty, slightly overflowing on left front.

Heart vessel obstructed: Palpitations, shortness of breath with inability to lie down, depression, mental restlessness,flustered feeling, feeling of oppression in the chest, stabbing pain in the heart region that comes and goes and may radiate to the upper back or shoulder, pain aggravated by exposure to cold and alleviated by heat, expectoration of phlegm, feeling of heaviness, dislike of speaking, cold hands, sighing, Purple lips face and nails. Tongue:  Purple overall or only on sides and chest area, swollen with a sticky coating. Pulse:  Wiry, choppy or knotted. Slippery if phlegm prominent

EMPTY-FULL PATTERNS

Heart-Blood Stasis: pain in the heart region, cyanosis of lips, purple tongue.

 

PERICARDIUM PATTERNS:

Heat in the pericardium: fever at night, delirium, tongue red without coating.

Blood deficiency of the pericardium: discomfort of chest, palpitations, insomnia

Pericardium-fire: palpitations, feeling of tightness in the chest, thirst, bitter taste, insomnia

Phlegm-fire harassing the pericardium: feeling of oppression and pain of the chest, all the mental symptoms.

Qi stagnation in the pericardium: feeling of distension in the chest, palpitations

Blood stasis of the pericardium: stabbing chest pain, purple lips, tongue.

  

LIVER PATTERNS:

Liver-Qi Stagnation: hypochondriac and chest distension, depression, moodiness, Wiry pulse.

Stagnant Liver-Qi Turning into Heat: distension, feeling of heat, irritability, red sides of tongue, Wiry pulse.

Rebellious Liver-Qi: Hypochondriac or epigastric distension, sighing, hiccup, nausea, vomiting, belching, "churning feeling in the stomach", irritability, in women distension of breasts. Tongue: color may be normal or with red sides. Pulse: Wiry, especially on the Liver and Stomach positions.

Liver-Blood Stasis: dark and clotted menstrual blood, purple tongue.

Liver-Fire Blazing Upwards: irritability, red face, red eyes, red tongue with yellow coating.

Damp-Heat in the Liver: fullness in abdomen and hypochondrium, feeling of heaviness, nausea, bitter or sticky taste, sticky yellow coating, slippery pulse.

Stagnation of Cold in the Liver Channel: hypogastric pain referring to scrotum, wiry-deep-slow pulse.

LIVER DEFICIENCY PATTERNS:

Liver-Blood deficiency: blurred vision, scanty periods, dull-pale complexion, pale tongue

Liver-Yin Deficiency: blurred vision, dry eyes, uncoated tongue.

Full/Empty PATTERNS:                  

Liver-Yang Rising: headache, irritability, wiry pulse.

Liver-Wind agitating within: There are four distinct types of Liver-Wind from different causes.  These are:

1)Extreme Heat generating Wind: high temperature, convulsions, stiff tongue.

2) Liver‐Yang rising deriving from Liver‐Yin deficiency:  tremor, facial tic, severe dizziness, tinnitus, headache, hypertension, dry throat, dry eyes, blurred vision, numbness or tingling of limbs, poor memory. Tongue: normal colored without coating. Pulse: wiry-fine

3) Liver-Yang rising deriving from Liver- and Kidney-Yin deficiency: tremor, facial tic, severe dizziness, tinnitus, headache, hypertension, dry throat, dry eyes, blurred vision, numbness or tingling of limbs, poor memory, backache, scanty urine, night sweats. Tongue: normal colored without coating. Pulse: wiry-fine

4) Liver‐Yang rising deriving from Liver‐Blood deficiency:  tremor, dizziness, tinnitus, headache, hypertension, dry throat, blurred vision, numbness or tingling of limbs, poor memory, insomnia. Tongue: pale and thin. Pulse: wiry-fine

Liver-Fire generating Wind: Irritability, propensity to outbursts of anger, tinnitus/deafness (sudden onset), temporal headache, dizziness, red face and eyes, thirst, bitter taste, dream‐disturbed sleep, constipation with dry stools, dark-yellow urine, epistaxis, hematemesis, hemoptysis. Tongue: Red body, redder on the sides, yellow coating, dry.

Liver-blood deficiency generating wind: fine tremor, dizziness, blurred vision, numbness or tingling of limbs, poor memory, insomnia, scanty periods. Tongue: pale and thin. Pulse: wiry-fine

LIVER COMBINED PATTERNS:

Rebellious Liver-Qi Invading the Spleen: symptoms: alternation of constipation and diarrhea, abdominal distension and pain

Rebellious Liver-Qi Invading the Stomach: Irritability, distension and pain in epigastrium and hypochondrium, fullness in epigastrium, sour regurgitation, belching, nausea, vomiting.

Liver-Fire Insulting the Lungs: symptoms: breathlessness, asthma, fullness of hypochondrium, headache, wiry pulse.

Heart- and Liver-Blood Deficiency: palpitations, dizziness, blurred vision, insomnia, poor memory, Pale tongue.

 
LUNG PATTERNS:

LUNG DEFICIENCY PATTERNS:

Lung-Qi Deficiency: shortness of breath, weak voice, empty pulse.

Lung-Yin Deficiency: dry cough, weak-hoarse voice, dry throat, night sweating.

Lung-Dryness: dry cought, dry throat, hoarse voice, dry tongue that is not red.

LUNG EXCESS PATTERNS:

Invasion of Lungs by Wind-Cold: aversion to cold, sneezing, floating pulse.

Invasion of Lungs by Wind-Heat: fever, aversion to cold, sore throat, floating-rapid pulse.                                                                                      

Invasion of Lungs by Wind-Water: sudden swelling of face, aversion to wind, floating-slippery pulse.

Lung Heat: cough, feeling of heat, thirst, red tongue with yellow coating.

Damp-Phlegm in the Lungs: chronic cough with profuse white sputum, thick-sticky white tongue coating.

Cold-phlegm in the Lungs: cough with expectoration of white‐watery sputum, aggravated by exposure to cold, cold feeling, cold hands, phlegm in the throat, dizziness, feeling of oppression and/or cold in the chest, feeling of heaviness, heaviness in the head. Tongue: swollen, wet with sticky white coating. Pulse: slippery-slow.

Phlegm-heat in the Lungs: cough, yellow or green sputum, thick-sticky yellow coating, rapid-slippery pulse.

Dry-Phlegm in the Lungs: dry cough with occasional expectoration of scanty sputum, feeling of oppression of the chest.

Phlegm-Fluids Obstructing the Lungs: cough with white-watery-frothy sputum.

COMBINATION PATTERNS:

Lung and Heart-Qi Deficiency: bright-white complexion, daytime sweating, depression, dislike of speaking, palpitations, propensity to catching colds, sighing, slight coughing, slight shortness of breath, spontaneous sweating, tiredness, weak voice. Tongue: Pale. Pulse: Empty, particularly in both Front positions.

 

SPLEEN PATTERNS:

SPLEEN EMPTY PATTERNS:

Spleen-Qi Deficiency: Poor appetite, tiredness, loose stools.

Spleen-Yang Deficiency: loose stools, feeling cold, cold limbs and tiredness

Spleen-Qi Sinking: bearing down sensation, weak pulse

Spleen not Controlling Blood: fine pulse, pale tongue and bleeding.

Spleen-Blood deficiency

FULL PATTERNS:

Cold-Dampness Invading the Spleen: abdominal fullness, feeling of heaviness, sticky tongue coating.

 

Damp-Heat Invading the Spleen: abdominal fullness, feeling of heaviness, sticky-yellow coating.

COMBINED PATTERNS:

Spleen- and Heart-Blood Deficiency: palpitations, insomnia, poor memory, pale tongue.

Spleen-and Lung- Qi Deficiency: poor appetite, tiredness and breathlessness.

Spleen-and Liver-Blood Deficiency: scanty periods, loose stools, blurred vision and pale sides of the tongue.

Obstruction of Spleen by Dampness with Stagnation of Liver-Qi: fullness of epigastrium, hypochondriac pain and a thick-sticky-yellow tongue coating.
 

KIDNEY PATTERNS:

Kidney-Yang Deficiency: cold in the back, abundant-clear urination, pale tongue, deep pulse

Kidney-Yin Deficiency: dry mouth at night, night sweating, red-peeled tongue.

Kidney-Qi Not Firm: dribbling after urination, nocturnal emissions without dreams.

Kidneys Failing to Receive Qi: shortness of breath on exertion, sweating, clear urination.

Kidney-Essence Deficiency: children: poor bone development; adults: weak knees, falling hair, weak sexual activity.

Kidney-Yang Deficient, Water Overflowing: edema especially of legs and ankles, cold feeling in legs and back, fullness and distension of abdomen, soreness of lower back, feeling cold, scanty-clear urination.

1. Kidney-Yang Deficient, Water overflowing to the heart: edema especially of legs and ankles, cold feeling in legs and back, fullness and distension of abdomen, soreness of lower back, feeling cold, scanty-clear urination, palpitations, breathlessness, cold hands.

2. Kidney-Yang Deficient, Water Overflowing to the lungs: edema especially of legs and ankles, cold feeling in legs and back, fullness and distension of abdomen, soreness of lower back, feeling cold, scanty-clear urination. Thin-watery-frothy sputum, cough, asthma and breathlessness on exertion.

Kidney-Yin Deficient, Empty-Fire Blazing: malar flush, mental restlessness, dry throat at night, feeling of heat in the afternoon, red-peeled tongue.

COMBINED PATTERNS:

Kidney-and Liver-Yin Deficiency: dry eyes, dry throat, night sweating, scanty menstruation, red-peeled tongue.

Kidney and Heart not Harmonized: palpitations, insomnia, night sweating, red-peeled tongue with redder tip and midline crack.

Kidney and Lung-Yin Deficiency: dry cough, dizziness, tinnitus, night sweating, and tongue without coating

Kidney and Spleen-Yang Deficiency: chronic diarrhea, feeling of cold in the back, deep-weak pulse.

 

STOMACH PATTERNS:

STOMACH EMPTY PATTERNS:

Stomach-Qi Deficiency: tiredness in the morning, uncomfortable feeling in the epigastrium, Empty pulse on the Stomach position.

Stomach Deficient and Cold: discomfort in the epigastrium which is better after eating, tiredness, cold limbs.

Stomach-Yin Deficiency: dull epigastric pain, dry mouth, tongue without coating "without root" in the centre.

STOMACH FULL PATTERNS

Stomach-Qi stagnation: Epigastric distention, belching, and irritability.

Stomach-Fire: burning sensation in epigastrium, thirst with desire to drink cold liquids, thick‐yellow coating, red tongue. Tongue: red center, yellow-dry coating. Pulse: rapid overflowing right middle position

 Stomach-Phlegm-Fire: feeling of oppression of epigastrium, less thirst, mucus in stools, expectoration of phlegm, mental derangement. Tongue: yellow-sticky coating. Pulse: slippery, overflowing, rapid.

Cold Invading the Stomach: sudden pain in epigastrium, vomiting, feeling cold, deep-tight pulse.

Stomach-Qi Rebelling Upwards: Nausea, belching.

Damp-Heat in the Stomach: epigastric fullness, feeling of heaviness, nausea, sticky-yellow coating.

Retention of Food in the Stomach: epigastric fullness, sour regurgitation, thick coating.

Stasis of Blood in the Stomach: stabbing pain in the epigastrium, vomiting of dark blood

Stomach-and Spleen-Qi Deficiency: Poor appetite, tiredness, epigastric discomfort.



SMALL INTESTINE PATTERNS:

EXCESS PATTERNS:

Full-Heat in the Small Intestine: abdominal pain, tongue ulcers, scanty-dark-painful urination.

Small Intestine-Qi pain: lower abdominal twisting pain, borborygmi, Deep-Wiry pulse.

Small Intestine-Qi tied: sudden severe abdominal pain, constipation, vomiting, Deep-Wiry pulse. This pattern is very similar to the previous one and it differs in so far as it is always an acute pattern.

Infestation of Worms in the Small Intestine

Small Intestine Deficient and Cold: abdominal pain, borborygmi, diarrhea.

 

LARGE INTESTINE PATTERNS:

EXCESS PATTERNS:

Damp-Heat in the Large Intestine: abdominal pain, diarrhea with mucus and blood in the stools.

Heat in the Large Intestine: dry stools, burning sensation in anus, thick-yellow-dry coating.

Heat obstructing the Large Intestine: constipation, abdominal pain, fever, thick-dry-yellow coating, Big pulse.

Cold invading the Large Intestine: sudden abdominal pain, diarrhea, feeling of cold.

Qi stagnation in the Large Intestine: abdominal distension and pain, bitty stools.

DEFICIENCY PATTERNS:

Large Intestine Dry: dry stools which are difficult to discharge, thin body.

Large Intestine Cold: loose stools, cold limbs, Fine pulse.

Collapse of Large Intestine: chronic diarrhea, prolapse ani.



GALL BLADDER PATTERNS:

EXCESS PATTERNS:

Dampness in the Gall-Bladder: hypochondriac fullness, feeling of heaviness and sticky unilateral coating.

Damp-Heat in the Gall-Bladder: hypochondriac fullness, bitter taste and thick-sticky yellow coating on the right side.

DEFICIENCY PATTERN:

Gall-Bladder Deficient: timidity, sighing, lack of initiative, indecision.

COMBINATION PATTERN:

Damp-Heat in Gall Bladder and Liver: hypochondrium fullness, bitter taste, nausea, sticky yellow coating on right side, genital skin rashes itching.

 

BLADDER PATTERNS:

EXCESS PATTERNS:

Damp-Heat in the Bladder: burning on urination, dark urine, difficult urination.

Damp-Cold in the Bladder: difficult urination, feeling of heaviness, turbid urine.

DEFICIENCY PATTERNS

Bladder Deficient and Cold: frequent-pale-abundant urination.
